Public School and Daycare Requirements

The state of Delaware requires the following for all children entering Kindergarten in the public school system or a licensed daycare :

  • A current physical & signed assessment form 
  • Up to date vaccines
  • Lead Test
  • TB Risk Assessment 

 

Adolescents entering high school in Delaware are now also required to have a physical assessment form completed before admission. In order for their physical to be considered current, it must  have been completed within the past year.
